Payments & Insurance


Patients are responsible for all payments. Payment is due at the time of the service. For your convenience, we accept: cash, checks, credit/debit cards.

Currently we only accept Seton Employee Insurance and private pay patients. We do provide a receipt that you can submit to your insurance company for reimbursement. Many insurance plans will reimburse you a percentage of the expenses. Please call your insurance prior to your appointment, to inquire about your percentage of coverage.

Cancellation Policy
When you make an appointment, the provider's time is blocked for you. If you need to cancel your appointment, please let us know 48 hours before your appointment time. Otherwise, you will be billed for the full price of your appointment.
